Current diagnosis

From a qualified provider — the condition must be documented now, not just historically.

Medication list

Current prescriptions related to the condition.

Imaging / lab results

MRI, X-ray, CT, blood panels — whatever supports your diagnosis.

Service treatment records (STRs)

Anything documenting the in-service event, injury, or symptom.

Service personnel records

DD-214, deployment history, MOS — to place you at the event.

VA medical records

Notes from any VA provider visit.

Private medical records

Any non-VA provider that treated you for the condition.

Prior VA decision letter (if any)

Needed for appeals and supplemental claims.

Lay statement (yours)

Your own description of onset, symptoms, frequency, and impact on daily life.

Buddy statement

A fellow service member who witnessed the in-service event or your symptoms.

Spouse / family statement

Especially powerful for PTSD, sleep apnea, and mental-health-adjacent claims.

Nexus letter

Doctor's opinion using 'at least as likely as not' language with medical rationale.

Completed DBQ (private)

Disability Benefits Questionnaire filled out by your own provider.

C&P exam notes

Once the VA's C&P exam happens, request a copy of the examiner's report.
